I think it just pertains to unique quest items. Such as, Madesi wants gold ore but gold ore is not unique so it doesn't stick in your inventory. EDIT: even though alto wine contradicts the above :) It really depends on the radiant quest itself. If it's asking for a mundane item, it will almost-but not completely always-tag it as important.
