First off, you leave work at whatever time you want. You never request to leave early. You miss time THEN WITHIN 7 DAYS you put in the amount of ppto you need to use. In your case you need to put in for 5 hours of ppto since you're not taking a lunch. Once you do it'll be automatically approved.
